Description:Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.We are currently seeking a Full-Time Case Manager. The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness to ensure their needs are addressed. The ACT Case Manager is accountable for carrying out the case management processes of problem identification, assessment, planning care, interventions, evaluation of client response, and revision of plan of care, as appropriate, in accordance with ACT fidelity requirements. The Case Manager works with the client in the community, home environment, and at agency locations. Works hours/shifts as posted.Essential Duties and Responsibilities:

  • As part of the ACT Treatment team, assesses client strengths and needs, conducts comprehensive care coordination, and contributes to the development of an individualized treatment plan delineating specific actions, and assumes responsibility for interventions.
  • Participates in daily ACT team meetings for planning, prioritizing and implementing daily tasks and assignments.
  • Monitors and evaluates client progress toward goal attainment. Reviews and modifies treatment plans as needed.
  • Maintains contact with the client as indicated in the treatment plan or as needed to ensure stability.
  • Provides active outreach as necessary, including regularly conducting field-based work, crisis intervention and management, and acquisition of appropriate community supports.
  • Links and/or refers clients to other services, programs, and agencies as needed. Assists in assuring clients obtain and retain applicable entitlements. Serves as an advocate for clients. With appropriate releases, communicate with family members and significant others as indicated.
  • Accurately documents service provision and billing information in accordance with agency timelines to assure reimbursement and compliance with fidelity requirements.
  • Provide on-call coverage during evening and weekend hours as required of members of the ACT team.
  • Perform other duties as assigned. Requirements: Specific/Individual Competencies:
    • Professional approach to all functions and relationships with all levels of staff, clients, other agencies, and the public.
    • Demonstrate knowledge of ACT program and ability to implement care within the framework of the fidelity model and agency policy and procedure.
    • Abides by the agency's code of ethics.
    • Knowledge of community resources, hospitals, community mental health services, and other appropriate referrals for use when indicated
    • Training requirements include CPR, CPI, new hire, Sanctuary and annual boosters in addition to any required trainings assigned during employment
    • Must be able to safely drive-in all-weather conditions in support of client needs
    • Works effectively and collaboratively within the multidisciplinary team including providing psychoeducation groups as needed.
    • Demonstrates excellent written and oral communication skills. Organizational Competencies:

Those eligible to drive company vehicles must have a valid driver's license and be eligible for coverage as defined by the agency commercial insurance carrier. Those who drive personal vehicles during business must be able to provide proof of insurance.
        • Bachelor's degree in related field required
        • Must have at least 2 years of experience working with severe and persistent mentally ill population
        • QHMS certification required (at minimum) throughout employment
        • CDCA preferred

Employee has contact with a wide range of people including some exhibiting anger and frustration under stressful conditions. Participants may be drug or alcohol-affected and in need of treatment.
