Job Description:
Job Title: Supervisor - Case Management Job Specialty: Case
Management Job Duration: 6 months Shift: Day - 5x8 - 8:00 AM - 4:30
PM, 40 hours per week Guaranteed Hours: 40 hours Experience: -
Minimum of 2 years of current acute care case management leadership
experience required. - Must have recent acute care case management
experience working full-time in a hospital setting. - Must have
inpatient hospital case management experience. - Must have
discharge planning and utilization management experience. -
Experience in leading or managing capacity is required. - Inpatient
psych experience is not suitable. License: RN or LMSW with a valid,
unrestricted State of Michigan license. Certifications:
Certification in Case Management (CCM) from the Commission for Case
Management Certification (CCMC) preferred. Must-Have: - Experience
with Epic and Careport. - Acute care case management leadership
experience. - Ability to mentor new hires and provide guidance on
difficult conversations with MDs and families. - Ability to counsel
staff on complex discharges. - Experience working in a hospital
setting following the height of COVID-19. - Hospital size of
previous experience does not matter. Job Description: - Responsible
for daily operations of case management activities in a specific
operational unit or department. - Conduct evaluations of
competency-based performance of staff members. - Ensure the
timeliness and effectiveness of each individual case manager---s
complex care interventions. - Monitor patient and provider
experience, process and outcome reports, and quality of
documentation through auditing. - Provide constructive feedback to
each case manager and collaborate on professional development
goals. - Serve as a resource to case managers caring for patients
with complex social, emotional, and medical needs. - Collaborate
with the Manager or Director to build, monitor, and evaluate
collaborative work processes to implement and optimize case
management best practices. - Collaborate with the Manager or
Director to develop educational curriculum and workflow tools. -
Provide leadership and guidance to staff for performance
improvement. - Lead workflow enhancement activities. - Plan and
organize orientation and continuing education opportunities for
staff. Candidates must be local only and willing to work as
personnel managers rather than case managers but need to have prior
case management experience for mentoring purposes. Team
transitioning to dyad model with Social Workers (SWs) and
Registered Nurses (RNs) functioning at the top of their
licenses.
