Job Description:

The Behavioral Health Case Manager is responsible for coordinating all services necessary to support the recovery and independence of individuals with behavioral health needs. This includes providing clinical and treatment plan consultation, ensuring compliance with agency standards, and assisting individuals in improving their quality of life and ability to function within the community. The position also involves implementing goal-oriented and individualized strategies for obtaining services and supports, while promoting recovery-based, person-centered, and culturally competent practices. Responsibilities:

• Facilitate the implementation of Individual Plan of Service
• Perform comprehensive, ongoing psychosocial assessments
• Provide community based services and coordinate consumer intakes, discharges, and transfers
• Communicate with all service providers in Treatment Plan and facilitate hospitalizations
• Coordinate and monitor activities such as job, school, volunteerism, and family caretaking
• Provide referrals to appropriate community resources Requirements:
  • Bachelor's degree in a human services field from an accredited institution
  • LLBSW, LBSW, LLMSW, LMSW preferred
  • Prior case management experience in mental health or rehabilitation setting highly preferred
  • Valid driver's license Full-time position with flexibility in shift scheduling.
